Eligibility & Enrollment
Employee Eligibility
Any active, regular, full-time employees working a minimum of 30 hours per week are eligible for all benefits upon date of hire.
Dependent Eligibility
- Your legal spouse or domestic partner*;
- Your dependent children (including your step-children and legally adopted children) up to age 26;
- Dependent children of any age who are incapable of self-support due to a disability
*Employees must provide a copy of a government-issued Marriage Certificate or a Declaration of Domestic Partnership Form at the time of enrollment. Documentation must be received before elections can be approved
When and How You Can Enroll
New hires can enroll within 30 days of their hire date through ADP WorkForce Now. Benefit elections become effective on the date of hire and remain in effect until CareDx’s annual Open Enrollment period held in the fall. Outside of annual Open Enrollment, the only other time you are permitted to make a change in your benefit enrollment elections is if you experience an IRS qualifying life status change. CareDx plans run for a January 1 – December 31 plan year.
You have 30 CALENDAR DAYS from your date of hire to enroll in benefits.
Changes in Benefit Elections
Your benefit elections are binding for the entire plan year. Changes are only permitted during Open Enrollment or mid-year if you experience an IRS Qualifying Life Event. If you experience a qualifying life event (see below), you may request a change to your benefits within 30 days of the event through ADP WorkForce Now. For assistance or to coordinate changes due to a qualified family status change, please contact HR Ops.
Qualifying events include (but aren’t limited to):
- Marriage or divorce
- Change in spouse’s employment status
- Birth or adoption of a child
You must provide proof of your Qualifying Life Event to HR Ops within 30 calendar days of the event to make changes to your benefit elections.
Enrolling in Benefits
ADP WorkForce Now is our online self-service benefits administration system. With just a few clicks you can enroll and make changes to your benefits, designate beneficiary(ies) for Life/AD&D insurance, find plan summaries, evidence of coverage booklets, and more. Be sure that all of your dependent information is accurate and complete. If you are enrolling new dependents, you will need their Social Security numbers and dates of birth.
